Steps to Become a New Patient

  1. Review our Office Policies page to see if we are the right fit for your child(ren).
  2. Ensure our providers accept your insurance plan.
  3. For HMSA HMO, and HMSA QUEST plans: one of our providers must be the assigned PCP (currently only Myrtle Parel, APRN is accepting new patients).
  4. For Tricare Prime plans: either Dr. Theresa Wee or Dr. Jordan Arakawa, must be assigned as the PCM (Primary Care Manager).
  5. All other HMO and Quest Plans: one of our providers must be the assigned PCP.
  6. Become acquainted with your insurance plan and benefits to understand not only your copayment, coinsurance and/or deductibles, but also your coverage (IE: well exams/immunizations).
  7. Have your child(ren)'s Medical Records/immunization records sent to our office for review.
  8. Once the records are received, our office will call you to schedule an appointment.
  9. Complete the New Patient Registration Form (must be completed by a natural parent or legal guardian).
  10. We encourage patients to fill out the forms before their visit to save time. But they can fill out the forms at the office if they prefer.
  11. For first-time appointments, the child must be accompanied by a natural parent or legal guardian (must provide legal paperwork).
  12. Provide your picture ID and insurance card.

Please note: We must be able to verify that one of our health care providers is assigned as the child’s PCP before they can be seen. If we're unable to verify this information, your child will need to be rescheduled until this process is complete.
